Family Check-Up Privacy Policy


Protecting your private information is our priority. This Statement of Privacy applies to your use of Family Check-Up® Online and your interaction with Northwest Prevention Science Inc. (“NPS”) and governs our data collection. For the purposes of this Privacy Policy, unless otherwise noted, all references to Family Check-Up® Online refer to all websites and online software services offered by NPS (collectively, the “Site”).

Before using the Site, you should carefully consider the data practices described in this statement, your own (or your organization’s) data requirements and practices, and you should not use the Site unless you consent to these practices and they are consistent with your own (or your organization’s) requirements.

How do we collect your information?

Your personal information is collected when you enter it into a form on the Site, when you navigate the Site, or when you voluntarily provide it to us in any other electronic means. NPS may collect personally identifiable information, such as your name, for the purpose of making use of the Site or providing services you request. Information about your computer hardware and software may be automatically collected by NPS. This information can include: your IP address, browser type, domain names, access times and referring website addresses. This information is used for the operation of the Site, to maintain quality of the Site, and to provide general statistics regarding use of the Site and NPS’s products and services.

How is this information used?

NPS does not sell, rent or lease its customer lists to third parties NPS collects and uses your personal information to operate its website(s) and deliver the services you have requested. NPS may keep track of the websites and pages you visit within the Site in order to determine what NPS services are the most used. NPS stores personal information in the cloud and the servers of our service providers. The servers of our trusted service partners are located in the United States. We reserve the right to share or disclose your personal information when we reasonably determine, in our sole discretion, that the disclosure of such information is necessary or appropriate to: (a) conform to the edicts of the law or comply with legal process served on NPS or the Site; (b) protect and defend the rights or property of NPS; (c) enforce a contract or agreement between you and NPS, and, (d) act under exigent circumstances to protect the personal safety of users of NPS, or the public. In addition, your personal information may be transferred to a third party as a result of an asset sale, acquisition, merger, reorganization or other change of control or if we sell, merge or transfer any part of our business, part of the sale may include your personal information.

Use of Cookies

The NPS website may use “cookies” to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a web page server. We respect your privacy and give you an opportunity to opt-out of receiving announcements of certain information. You may also contact us to request that we remove or correct your information, or you may request a copy of your information be provided to you.
